In Singapore, the government rewards those who stay opted in organ donation with higher transplant priority, while opt-outs face long-term disadvantages. Supporters see this as mutual responsibility, while others see inequality. Is this an ethical incentive or subtle punishment? How should fairness be defined here?

I think this is ethical incentives. If you aren't ready to "give", how can you justify your desire to "receive." Since donation is after your "death" I don't see why you should opt out from donation.
