Someone who worked in the fields all day, then when they went out for a beer or a meal had to scrub the grime off, leaving their necks red!!
Could also be because working outdoors burned their necks.
Take your pick, but it seems to have originally been a description of someone who worked hard, which has now become something of a disparaging term!!
