If you don't hold onto it and it comes off as you dismount, you can have a problem.

Condoms are not really 99% effective imo, they smell bad, feel bad, and aren't as easy to use as people make them out. If pregnancy is your main concern (and not STD's) pills, patches, rings, or inserts are your best bet.

If you aren't capable of getting pregnant, yet you are worried that a pregnancy has occurred while using a condom, then you are hearing about the condom use second hand. So you can't be certain that it was indeed used correctly or that it didn't break.
The condom would have to be inspected to see if it indeed break or not. And I can tell you from experience, once a condom is taken off, it's just chucked in the trash, you don't hold it up and squeeze it to see if there is any leakage.
Sounds like this guy, who ever he is, has gotten a call from an old flame who says she's preggo... Let's hope this guy was smart enough to flush his used condom, because if she wants to be pregnant by him (for money, support, get him to come back to her) she could have emptied the used condom into her to get what she wants... and don't think that doesn't happen, it does.
