Actually,the idea for came from a comment that the director made when he and one of friends were watching a news report about a child molester. He said they should stitch his mouth to the ass of a very fat truck driver as a punishment, and the idea kept growing from there.

LOL But if you ever watch the first movie again,you'll notice that Dr Heiter's first victim for his project was a fat truck driver just like the director's original idea. Unfortunately for the fat truck driver he was too big to be conjoined with the girls and had to be killed. :willy_nilly:
