I heard about this kid before I saw this thread. I read a story about it a while back. If I remember correctly, that's actually what he was doing. He had to pee or was hungry or something that most adults would just ignore during filming, but since he's a child, he didn't really know any better, and he was trying to make a subtle motion to get the attention of the director/crew so they would cut and let him go do whatever it was he needed to do. It just ended up getting caught on film and wasn't really noticed before the film was finished and released.

Edit: There's a discussion on the Snopes.com forum about this very topic actually. Most of the belief is that the child just had to pee, and that the people in the editing department either didn't catch this motion in time to remove it or re-shoot it, or that since multiple takes are done during films, this may have been the best take out of, for example, six other ones that weren't as good, and the director just had to deal with this hand motion. Not to mention, in the actual film, the camera doesn't zoom in that far to show the child's hand motion, so 90% of people don't, haven't, or never will even notice this little hand motion while watching the film.
